• Guaranteed Hours: Facility can call off UP TO one shift per pay period (every two weeks)
• Travel Requirements: Traveler is expected to have their own transportation. Occasional maximum radius of 50 miles.

This is a Home Health Physical Therapist travel/contract opportunity based in Medford OR and surrounding areas. The role is with a home health facility operating under the Providence Health system. Clinicians will work a standard 5x8 day shift schedule; this facility cannot accommodate 4x10 or 3x12 schedules. Therapists may visit up to 6 patients per day with a standard productivity expectation of 30 productivity points. The system does a good job grouping clients to minimize drive and windshield time with an occasional maximum radius of 50 miles.

• Visit up to 6 patients per day in a home health setting within the Medford OR and surrounding areas
• Meet a standard productivity expectation of 30 productivity points
• Utilize EPIC/Oasis for charting and documentation
• Provide skilled physical therapy services to home health patients in a 1:1 ratio

• Proficiency with Oasis documentation (required candidates without Oasis experience must be called out at time of submission)
• Ability to manage a home health caseload with drive time across a geographic service area
• Valid drivers license and personal reliable transportation

• Minimum 1 year of skilled Home Health experience required
• First-time travelers are not accepted for this position

• PT License required
• BCLS (Basic Cardiac Life Support) certification required
• Pending license accepted

• This facility cannot accommodate 4x10 or 3x12 schedules; 5x8 days only
• Facility requires the primary series COVID vaccination in addition to one booster
• Oasis experience is required please call out at submission if the traveler does not have Oasis experience
• RTO restrictions: Confirm with hiring manager
• Ratios: 1:1 (may visit up to 6 patients per day; 30 productivity points standard)
