Duties

  • Develop and implement individualized care plans that incorporate evidence-based physiotherapy techniques such as trigger point therapy and orthopedics rehabilitation.

  • Deliver hands-on therapy sessions focusing on improving patient outcomes through therapeutic exercises manual therapy and patient education.

  • Coordinate discharge planning with multidisciplinary teams to ensure seamless transition from inpatient or outpatient settings to home care.

  • Monitor patient progress regularly adjusting treatment strategies as needed to meet evolving health goals.

  • Maintain accurate documentation of treatments progress notes and care plans within electronic medical record (EMR) systems while adhering to healthcare regulations.

  • Educate patients and caregivers on injury prevention activity modifications and self-management strategies to support ongoing recovery at home.

Skills

  • Strong understanding of medical terminology physiology and human anatomy essential for accurate assessment and treatment planning.

  • Proven experience in inpatient hospital settings outpatient clinics or home health environments with a focus on patient care continuity.

  • Expertise in discharge planning and managing care transitions across various healthcare settings.

  • Familiarity with EMR systems for efficient documentation and communication of patient information.

  • Knowledge of pediatric geriatric orthopedics geriatrics and community care practices to address diverse patient needs effectively.

  • Ability to apply physiotherapy techniques such as trigger point therapy within treatment protocols.
