Job Summary
Sign-On Bonus: $10000
Shift: Full-time Days Monday - Friday for 80 hours per pay period.
The Physical Therapist-Home Care delivers evidence-based therapy in the patients home conducting comprehensive evaluations and developing individualized plans of care in collaboration with physicians and the interdisciplinary team. This role provides skilled interventions educates patients and caregivers and ensures timely accurate documentation all aligned with clinical best practices to improve function safety and quality of life while achieving strong outcomes and an exceptional care experience.

- Perform comprehensive patient assessments in the home setting
- Develop and implement individualized treatment plans focused on functional goals
- Provide therapeutic exercises manual therapy and mobility training
- Educate patients and caregivers on treatment plans safety and home exercises
- Collaborate with physicians nurses and interdisciplinary care teams
- Monitor patient progress and adjust care plans as needed
- Maintain accurate and timely clinical documentation
- Ensure compliance with home health regulations and agency standards
Education:
- Graduate of an accredited Physical Therapy program (DPT or equivalent)
Experience:
- One year of prior experience caring for similar patient population as to be assigned
Certificate/License:
- Licensed as a Physical Therapist in the state of Iowa. You will be required to obtain a WI and IL license within 12 months of employment
- Must hold a valid driver license and automobile insurance
