Physical Therapist Full Time

All Heart Home Care is seeking a compassionate and skilled Physical Therapist (FT) to join our home health team in the Murfreesboro area. This role offers flexible as-needed scheduling while providing personalized one-on-one care to patients in the comfort of their homes.

Key Responsibilities:

• Perform in-home evaluations to assess patient needs and develop individualized treatment plans
• Provide skilled physical therapy services to improve mobility strength balance and overall function
• Monitor patient progress and adjust treatment plans as appropriate
• Educate patients and caregivers on home exercise programs safety and injury prevention
• Complete accurate and timely clinical documentation in compliance with company policies and state regulations
• Coordinate care with physicians nurses case managers and other members of the healthcare team
• Travel to patient homes within assigned service areas

Qualifications:

• Doctor of Physical Therapy (DPT) or equivalent from an accredited program
• Active Physical Therapist license in the state of Tennessee
• Current CPR certification
• Valid drivers license reliable transportation and current auto insurance
• Strong clinical assessment and communication skills
• Ability to work independently and manage time effectively
• Previous home health experience preferred but not required
