Ongoing professional development and career growth supportKey Responsibilities Conduct comprehensive Physical Therapy evaluations in clinical and community-based settings
Develop individualized treatment plans based on patient goals and functional needs
Participate in interdisciplinary care planning and team meetings
Deliver therapeutic interventions to improve strength mobility balance and overall function
Provide gait training therapeutic exercise functional mobility training and activities of daily living instruction
Educate patients caregivers and staff on therapeutic techniques and equipment use
Supervise and mentor Physical Therapy Assistants and support staff as needed
Maintain accurate and timely clinical documentation
Bachelors degree in Physical Therapy required; Masters or Doctorate preferred
Active Physical Therapy license or eligibility for licensure
Experience working with older adults medically complex patients or rehabilitation populations preferred
Strong communication and collaboration skills
CPR and First Aid certification (or ability to obtain within required timeframe)Ideal candidates will have experience in outpatient inpatient acute care rehabilitation skilled nursing or geriatric settings and a passion for delivering compassionate patient-centered care.
