About the Role
We are seeking a Physical Therapist for an outpatient rehabilitation assignment in Carlinville Illinois. This role is ideal for a skilled therapy professional who can evaluate patients develop treatment plans and support functional recovery in a critical access hospital setting.
Responsibilities
Evaluate patients and create individualized physical therapy treatment plans
Provide outpatient therapy services to improve mobility strength and function
Monitor patient progress and adjust care plans as needed
Maintain accurate and timely documentation according to facility requirements
Follow billing procedures and upload all required documentation as instructed
Collaborate with providers and rehab team members to support quality outcomes
Requirements
Active Illinois Physical Therapist license required
Minimum 1 year of Physical Therapist experience
Outpatient rehabilitation experience preferred
CPSI/Evident EMR experience preferred
Must review and follow all uploaded assignment requirements
Holiday coverage may be required
