As a Home Health Physical Therapist Assistant youll work alongside a supervising Physical Therapist to help patients regain strength improve mobility reduce pain and achieve the goals that matter most to them.

• Provide skilled physical therapy treatments under the direction of a Physical Therapist
• Implement individualized treatment plans
• Monitor patient progress and communicate findings to the supervising Physical Therapist
• Educate patients and caregivers on therapeutic exercises mobility techniques and fall prevention
• Assist patients in improving strength balance endurance and functional mobility
• Collaborate with physicians and interdisciplinary team members
• Complete timely and accurate clinical documentation

• Active Virginia Physical Therapist Assistant (PTA) License Required
• Valid Drivers License and Reliable Transportation
• Strong Communication and Documentation Skills
• Home Health Experience Preferred but Not Required
• Passion for Delivering Exceptional Patient-Centered Care
