Job Summary

Navitas Healthcare LLC is seeking an experienced Physical Therapist Assistant (PTA) for a 13-week travel assignment. This role focuses on providing skilled home health rehabilitation services to patients in residential settings while supporting individualized treatment plans and functional recovery goals.

The ideal candidate will have prior skilled home health experience strong communication skills and the ability to work independently while managing daily patient visits across the Portland service area.

Key Responsibilities
  • Assist Physical Therapists in implementing individualized treatment plans
  • Provide therapeutic exercises and rehabilitation interventions in patient homes
  • Monitor patient progress and report status changes to supervising therapists
  • Educate patients and caregivers regarding exercises mobility and safety techniques
  • Maintain accurate and timely clinical documentation using EPIC and Rover systems
  • Support functional mobility strength balance and independence goals
  • Follow home health protocols infection prevention standards and safety guidelines
  • Coordinate care effectively with interdisciplinary healthcare teams
  • Manage daily travel between patient homes within assigned territory
Required Qualifications
  • Active Physical Therapist Assistant (PTA) license required
  • Minimum 1 year of skilled Home Health PTA experience required
  • Current BCLS certification required
  • Ability to work independently in home health settings required
  • Valid drivers license and reliable transportation required
  • Strong patient care and communication skills required
