Job Description:

Elara Caring is hiring a compassionate Physical Therapist Assistant to serve home health patients throughout lower Fairfield County.

The territory extends from Norwalk to the New York state line including Stamford and Greenwich.

As a Home Health PTA you will provide personalized therapy in patients homes under the supervision of a Physical Therapist. You will help patients improve their strength mobility balance safety and confidence while building meaningful one-on-one relationships.

• Provide physical therapy treatments according to the established Plan of Care
• Work under the direction and supervision of a licensed Physical Therapist
• Help patients improve mobility strength balance endurance and independence
• Monitor patient progress and promptly report changes in condition to the supervising Physical Therapist
• Review the Plan of Care before each visit
• Provide treatment according to physician orders and established clinical standards
• Teach patients and caregivers how to safely complete home exercise programs
• Educate patients on the safe use of walkers canes wheelchairs prosthetics orthotics and other assistive devices
• Reinforce patient goals and encourage participation in therapy
• Complete accurate and timely clinical documentation
• Collaborate with nurses therapists physicians and other members of the care team

• Graduate of an accredited Physical Therapist Assistant program
• Current and unrestricted Connecticut Physical Therapist Assistant license
• At least six months of experience as a Physical Therapist Assistant preferred
• Strong communication organization and documentation skills
• Ability to work independently in patients homes
• Willingness to travel throughout Norwalk Stamford Greenwich and the assigned coverage area
• Must possess a valid drivers license maintain an acceptable driving record OR have reliable transportation to travel between patient homes and assigned work locations.
• Must be able and willing to travel 50%
• Ability to frequently lift push pull and support up to 50 pounds including positioning or transferring patients and moving equipment
