As a Home Health Physical Therapist Assistant (PTA) with Elara Caring youll provide personalized therapy to patients throughout the Easton territory. Working under the direction of a licensed Physical Therapist youll turn individualized treatment plans into meaningful real-life progress.

As a Physical Therapist Assistant you will:

• Carry out individualized treatment plans established by the supervising Physical Therapist.

• Help patients improve strength balance mobility coordination and functional independence.

• Guide patients through therapeutic exercises and activities based on their personal goals.

• Teach patients and caregivers how to safely continue prescribed exercises between visits.

• Provide instruction on the proper use of walkers canes wheelchairs prosthetics orthotics and other assistive devices.

• Monitor each patients progress and promptly report changes in condition or response to treatment.

• Complete accurate and timely clinical documentation.

• Follow physician orders the established Plan of Care state practice standards and organizational policies.

• Collaborate with the Physical Therapist and other care team members to support safe successful outcomes.

• Graduation from an accredited Physical Therapist Assistant program.

• Current unrestricted PTA license in the state of practice.

• Six months of Physical Therapist Assistant experience preferred.

• Home health experience is a plus.

• Valid drivers license dependable transportation
