Physical Therapist Assistant PRN Float Nashville TN

All Heart Home Care is seeking a dedicated and compassionate Physical Therapist Assistant (PTA) to join our home health team as a PRN Float in the Nashville area. This flexible as-needed role offers the opportunity to provide personalized one-on-one care to patients in the comfort of their homes while supporting multiple locations as needed.

• Provide in-home physical therapy services under the direction and supervision of a licensed Physical Therapist
• Assist in implementing individualized treatment plans to improve patient mobility strength balance and overall function
• Monitor patient progress and report any changes to the supervising Physical Therapist
• Educate patients and caregivers on prescribed exercises safety protocols and home care techniques
• Maintain accurate and timely clinical documentation in compliance with company policies and regulatory requirements
• Collaborate with the interdisciplinary team to ensure coordinated and effective patient care
• Travel to patient homes throughout the Nashville service area as needed

• Graduate of an accredited Physical Therapist Assistant program
• Active Physical Therapist Assistant license in the state of Tennessee
• Current CPR certification
• Valid drivers license reliable transportation and current auto insurance
• Strong communication and interpersonal skills
• Ability to work independently and adapt to varying patient needs and locations
• Previous home health experience preferred but not required

• PRN (as needed)
• Flexible scheduling based on patient needs and coverage demands
