Schedule: Day shift
Contract: 13 weeks
Guaranteed Hours: To be confirmed
Experience: SNF or post-acute experience preferred

Responsibilities

• Evaluate and treat patients in skilled nursing and post-acute settings
• Develop individualized treatment plans based on patient needs and goals
• Provide therapy to improve mobility strength balance function and independence
• Collaborate with interdisciplinary care teams for coordinated patient care
• Document treatment progress and outcomes accurately and timely
• Participate in care conferences and quality improvement efforts
• Support a patient-centered and compassionate therapy environment

Requirements

• Active California Physical Therapist license required
• Skilled nursing or post-acute experience preferred
• Strong clinical assessment and treatment planning skills
• Excellent communication and documentation skills
• Dependable flexible and team-oriented
