Location: Apple Valley California
Contract Duration: 13 Weeks
Schedule
- 5x8 Hour Shifts
- Tuesday Saturday
- 9:00 AM 5:30 PM
Position Overview
We are seeking a skilled Acute Care Physical Therapist to provide evaluation treatment and rehabilitation services to patients across a variety of acute care settings. The ideal candidate will have experience working with medically complex patients and developing individualized treatment plans to maximize functional mobility and independence.
Required Qualifications
- Active California Physical Therapist License
- Minimum 1 year of Acute Care Physical Therapy experience
- BLS Certification
- First-time travelers accepted
Key Responsibilities
- Evaluate patients and develop individualized treatment plans
- Establish measurable goals and monitor patient progress
- Perform reassessments and modify treatment plans as needed
- Provide patient and family education regarding rehabilitation and home exercise programs
- Coordinate discharge planning and continuity of care
- Document evaluations treatments progress notes and care plans accurately
- Collaborate with physicians nurses case managers and interdisciplinary teams
