As a Home Health Physical Therapist you will:

• Evaluate patients mobility strength balance and functional abilities in their homes

• Develop individualized treatment plans based on each patients needs and personal goals

• Provide hands-on therapy to improve movement safety and independence

• Adjust treatment plans as patients progress and their needs change

• Educate patients and caregivers on exercises equipment fall prevention and safe mobility

• Collaborate with physicians nurses therapists and other members of the care team

• Guide support staff in helping patients achieve their functional goals

• Complete accurate and timely clinical documentation

• Serve as a trusted therapy resource for patients families colleagues and the community

• Graduate of an American Physical Therapy Association-approved physical therapy program

• Current unrestricted Physical Therapist license in the state of practice

• Dependable vehicle valid drivers license and current automobile insurance

• Ability and willingness to travel throughout the assigned coverage area

• Ability to work independently while remaining connected to a collaborative care team

• Sit stand bend lift and move intermittently and be able to lift 50100 lbs.
