Job Summary and Responsibilities

As a Pharmacy Technician Medication History you will provide critical support in gathering and accurately documenting patients medication histories profoundly impacting medication safety and comprehensive patient care.

Every day you will expertly interview patients or their caregivers access various data sources review current and home medications and meticulously record precise medication histories in the electronic health record collaborating closely with clinical teams.

To be successful you will demonstrate outstanding attention to detail strong communication skills and a meticulous empathetic demeanor contributing significantly to medication reconciliation and preventing adverse drug events.

  • Collects and documents a patients allergies and a current accurate medication history
  • During medication history collection identifies prescription non-prescription (OTC) herbals implantable devices/pumps and any illegal substances
  • Clarifies incomplete or inaccurate information with the dispensing pharmacy the provider who wrote the prescription utilizing Controlled Substance Monitoring Database and Drug Claim databases (SureScripts DrFirst etc.)
  • Enters validated medication history information into the patients electronic medical record/patient chart in accordance with established procedures
  • Meets or exceeds Memorials quality control standards for accuracy and productivity based on completed medication histories. Consults with clinical pharmacists on any unresolved issues or those requiring a pharmacists clinical judgment
  • Utilizes software to Identify and follow up on any missed medication histories or patients with follow-up needs
Job Requirements

  • High School Graduate upon hire or
  • High School GED upon hire and
  • 3 years of Pharmacy technician experience in any setting and
  • Pharmacy Technician: WA upon hire


Preferred

  • Retail pharmacy experience and
  • Graduate of ASHP accredited Pharmacy Technician Program upon hire or
  • Associates Other or Equivalent Experience upon hire
