Pharmacy Technician Medication History

Purpose:

Under the supervision of a pharmacist the medication history technician (MHT) will obtain medication histories through interviews with the patient/family/care partner. The MHT will use technology to pull in external fill history from patientsï½ pharmacies.

Essential Responsibilities:

1. Obtains and clarifies patientï½s home medication list including prescription OTC and herbal products. Any bottles brought in will be promptly returned to the patientï½s family/care partner after all important information has been transcribed.
2. Responds to nurses physicians pharmacists or other requests to manage triage appropriately and ensure follow-up promptly regarding med history information.
3. Obtains patientï½s allergies & intolerances and communicates to appropriate caregiver for proper documentation into the electronic medical record (EMR) including symptoms reactions and severity.
4. Directs triages and communicates to appropriate caregiver all questions and inquiries from patient/family/care partner outside the scope of the MHT.
5. Identifies and participates in ongoing quality assurance and improvement processes to obtain an appropriate med history list.
6. Fulfills all compliance responsibilities related to the position.

8. Demonstrates regular reliable and predictable attendance.
9. Performs other duties as required

Required:

• High School Diploma or equivalent
• Minimum Experience: 1 year

Desired:

• Retail Pharmacy Technician experience.
• Computer skills ability to multi-task and work independently.

License Registration or Certification Requirements:

• PTCB Certification within 6 months
• License: Registered Pharmacy Technician with NY Board of Pharmacy within 6 months
