Position Summary

Coordinates and provides pharmaceutical care for patients collaborating with the interprofessional team to optimize safe effective medication use. Reviews and verifies orders manages drug therapy protocols educates staff/patients and supports quality and regulatory standards. Reports to the Director or Manager of Department.

Essential Functions

• Manages drug therapy protocols as part of an interdisciplinary team.
• Reviews interprets and evaluates medication orders to identify prevent and resolve drug-related problems.
• Provides drug information and education to medical staff healthcare professionals patients and pharmacy students.
• Documents interventions and outcomes for drug therapy quality improvement.
• Verifies medication orders for accuracy/completeness prior to entry; checks filled medications prepared by technicians; maintains proficiency with pharmacy automated systems and software.
• Manages patients in an outpatient anticoagulation clinic including point-of-care testing result evaluation dosing and scheduling.
• Participates in performance improvement and required continuing education.
• Performs other duties as assigned.

• Education: Doctor of Pharmacy
• License: Current license with the state Board of Pharmacy
• Certifications: BCLS required
