Salary:$47874 to $55055 annually

Schedule: Monday-Friday 8am-4pm (40 hr/week) *flexibility with schedule needed to meet programs needs*
What does a Person-Centered Plan Manager do
As a Person-Centered Plan Manager you will be responsible for providing dedicated support to Care Providers in fulfilling their responsibilities. Your key duties will include:
- . Conducting monthly home visits for program members and their Care Providers ensuring their well-being and compliance with regulations.
- Completing all necessary documentation accurately and promptly to maintain thorough records of each visit.
- Maintaining open lines of communication with assigned staff and coordinating with other service providers such as day programs and work programs to ensure comprehensive care for program participants.
- Making appropriate referrals to external resources when additional support is needed.
- Monitoring the home setting according to regulatory guidelines to ensure the safety and comfort of individuals with disabilities.
Qualifications and Skills:
- Must pass background checks and pre-employment screenings including TB test and CORI.
- Bachelors degree in social work human services or a related field (or equivalent experience).
- Basic computer skills using Microsoft Office email communication internet and documentation entry.
- Strong interpersonal and communication skills to effectively collaborate with program participants their Care Providers and a multidisciplinary team.
- Ability to prioritize tasks work independently and within a team and meet deadlines.
- Compassionate resourceful and eager to learn.
- Valid drivers license and reliable personal vehicles meeting state requirements.
