Role Scope:

The Physical Therapist Assistant (PTA) provideshigh-qualityrehabilitativesupport forparticipantsenrolled inthe HabitatHealthPACE program (Program ofAllInclusiveCare for the Elderly). Working under the supervision of a licensed Physical Therapist the PTA delivers therapeutic interventions that promote mobility safety independence and overall quality of life. Thisperdiemrole offers flexible scheduling while supporting participant needs across clinichome anddaycentersettingsin Sacramento.

Key Responsibilities

  • Therapeutic Interventions Implement individualized treatment plans including therapeutic exercise gait training balance activities and functional mobility tasksunder the supervision of the PT.
  • Participant Assessment Support Observe document and report participant progress functional changes and safety concerns to the supervising PT.
  • Interdisciplinary Collaboration Coordinate with nurses social workers occupational therapists and other PACE team members to ensure integratedpersoncenteredcare.
  • Home & Center-Based Care Provide therapy services in participant homes thePACE daycenter and clinic environments as needed.
  • Equipment & Safety Train participants and caregivers on safe use of assistive devices and mobility equipment;maintaina safe therapeutic environment.
  • Documentation Completeaccuratetimelyelectronic health record (EHR) documentation in compliance with PACE state and federal guidelines.

Qualifications

  • Accredited PTA Program graduate with an active California PTA license.
  • Current CPR/BLS certification.
  • Experience working with geriatric populations or inlongtermcare home health or PACE preferred.
  • Ability to work independently travel locally and adapt to changing participant needs.
  • Strong communication documentation and teamwork skills.

Schedule & Work Environment

  • Per diem / asneededtypically 8am to 5pm
  • Services delivered acrossHabitat Health PACE Centers (Sacramento South LA San Leandro West Covina)participant homes.

Compensation:

The anticipated base salary range for this position is $40.00 - $46.00 per hour. The final compensation offered will bedeterminedbased on factors including but not limited to the candidates qualifications skills experience education certifications geographical location and business needs. In addition this position is eligible for a comprehensive benefits package including medical dental and vision insurance;paid parental leaveshort- and long-termdisability coverage; life insurance; flexible spending accounts; a 401(k) savings planthat includes a match; paid time off; and companypaid holidays.
