As a Home Health Physical Therapist youll have the opportunity to provide care where it matters mostyour patients homes.
Youll help individuals recover from surgery illness injury hospitalization and mobility challenges while improving strength balance safety and independence.

- Complete evaluations reassessments and discharge visits
- Develop individualized treatment plans based on patient goals and needs
- Provide skilled physical therapy services in patients homes
- Educate patients and caregivers on mobility exercise programs fall prevention and safety strategies
- Collaborate with physicians and interdisciplinary team members
- Maintain timely and accurate clinical documentation
- Active Pennsylvania Physical Therapist License Required
- Valid Drivers License and Reliable Transportation
- Strong Clinical Communication and Documentation Skills
- Home Health Experience Preferred but Not Required
