As a Home Health Physical Therapist youll work directly with patients in their homes helping them improve mobility restore function prevent falls and regain confidence in their daily lives.

Youll have the flexibility to create a schedule that works for you while being supported by an experienced clinical team committed to your success.

• Complete evaluations reassessments and discharge visits
• Develop individualized treatment plans
• Deliver skilled physical therapy services in patients homes
• Educate patients and caregivers on mobility safety and recovery strategies
• Collaborate with physicians and interdisciplinary team members
• Maintain timely and accurate documentation

• Active Virginia Physical Therapist License Required
• Valid Drivers License and Reliable Transportation
• Strong Clinical Communication and Documentation Skills
• Home Health Experience Preferred but Not Required
