Human Touch Home Health is seeking a Physical Therapist (PT) for PRN home health opportunities throughout Carbon County Pennsylvania.

- Complete physical therapy evaluations in patients homes
- Develop individualized plans of care
- Provide therapeutic exercise gait balance transfer and mobility training
- Assess home safety and fall risks
- Educate patients and caregivers
- Monitor patient progress and adjust treatment plans
- Complete progress assessments and discharge planning
- Coordinate care with nurses physicians and other team members
- Complete accurate and timely documentation using WellSky
- Active Pennsylvania Physical Therapist License Required
- Graduate of an accredited Physical Therapy program
- Valid Drivers License and reliable transportation
- Strong clinical assessment and communication skills
- Ability to work independently in patients homes
- Home Health Experience Preferred but Not Required
- WellSky Experience is a Plus
