Per Diem Physical Therapist

Job Description:
We are currently seeking a dedicated and compassionate Physical Therapist (Per Diem) to join our home health team in Lynn and Dorchester MA. This is a flexible per-visit opportunity ideal for clinicians who enjoy working independently and providing high-quality care in patients homes.

Responsibilities:

• Provide physical therapy services to patients in their homes

• Evaluate patient needs and develop individualized treatment plans

• Complete accurate and timely documentation including OASIS assessments

• Educate patients and families on treatment plans and progress

• Collaborate with the interdisciplinary care team to ensure optimal patient outcomes

Qualifications:

• Active Physical Therapist (PT) license in Massachusetts

• CPR certification (current)

• OASIS experience required

• Home health experience preferred

• Reliable transportation and willingness to travel within assigned areas

• Strong communication and documentation skills

• English proficiency required
