Per diem Occupational Therapist

Our leading Home Health Agency is seeking a compassionate per diem Occupational Therapist for home visits in Norwalk and surrounding areas.

We offer offering skilled nursing rehabilitation and non-skilled services for the residents in the community that require support with ADLs that may have a physical and/ or developmental disability.

As a Occupational Therapist on our team you will assess patients create treatment plans provide exercises track progress and educate on injury prevention while collaborating with the healthcare team.

Qualifications:

• Licensed Occupational Therapist in Connecticut

• Reliable transportation and a willingness to cover Norwalk and surrounding areas

• Proven experience in Occupational therapy preferably in a home health or clinical setting

• Strong understanding of musculoskeletal and neurological conditions

• Excellent communication and interpersonal skills

• Ability to develop and implement individualized treatment plans

• Knowledge of adaptive equipment and assistive devices

Responsibilities:

• Evaluate patients physical conditions and create personalized treatment plans

• Develop and implement therapeutic exercises to improve mobility strength and function

• Monitor patient progress and adjust treatment plans as needed

• Educate patients and their families on exercises safety techniques and home care

• Collaborate with healthcare teams to ensure comprehensive care

• Maintain accurate and up-to-date patient records

• Provide guidance on injury prevention and rehabilitation strategies
