$50/hour (based on actual visit time)

Employment Type: Per Diem Field Visits throughout the Belmont Lakes Region and Concord areas

We are seeking experienced and compassionate Licensed Practical Nurses (LPNs) to join our growing home care team on a per diem basis. This position offers independence flexibility and meaningful one-on-one care for patients who prefer to remain in the comfort of their homes.

Service areas currently extend throughout the Lakes Region and Central New Hampshire with opportunities ranging from Laconia Belmont and Gilford south through Tilton Franklin and Concord.

The LPN will provide skilled nursing visits focused on promoting recovery maintaining stability and enhancing quality of life. This is an ideal role for nurses who thrive in autonomous settings and want flexible scheduling with competitive per-visit compensation.

Conduct in-home patient assessments medication management and wound care

Monitor and document changes in patient condition and report to supervising RNs or physicians

Educate patients and caregivers on treatment plans medications and home safety practices

Maintain accurate timely documentation using electronic charting

Collaborate with multidisciplinary care teams to ensure coordinated and effective patient care

Current and active LPN license in the State of New Hampshire required

Valid drivers license and reliable transportation

Prior experience in home care long-term care rehabilitation or community-based nursing preferred

Visits may be assigned throughout the Belmont Laconia Gilford Tilton Franklin and Concord areas based on your availability and preferred travel radius.
