Responsibilities:

  • Develops a P.O.C. that corresponds with the physicians orders through assessment of patients needs condition environment consultation with other health team members at time of initial visit and updates plan of care regularly as necessary and documents this care following Agency procedures
  • Assumes responsibility for continuity quality and safety of services delivered in compliance with State and federal regulations
  • Monitors and supports staffing to ensure adequate staffing of clients with appropriate skill level
  • Assists in the direction and coordination of clinical departments and branches
  • Promotes the philosophy and administrative policies of the Agency in an effort to ensure quality of patient care
  • Supervises all direct patient care services provided by clinical personnel in the patients home environment
  • Responsible for the implementation and evaluation of individual patient plan of care
  • Provides skilled nursing care that conforms with Agency patient care and general medical policies
  • Coordinates schedules of office and field clinical staff
  • Assists in proper maintenance of clinical records in compliance with local state and federal laws
  • Follows all privacy policies and maintains the confidentiality of protected healthcare information (PHI)
  • Supports personnel development including orientation in-service education and continuing education
  • Works with HR to ensure branch personnel files are maintained up to date
  • Facilitates coordination of care within the interdisciplinary team patient/family insurance providers physicians and referring agencies
  • Provides direction to subordinates in an effort to ensure quality compliance with Plan of Care and assessment and reassessment of patients needs and continuity of services
  • Provides supervision to all field clinical staff
  • Assists to make sure branch meets QAPI standards (including meeting minutes audits logs patient satisfaction reviews)
  • Ensures that Branch QAPI corrective measures are implemented
  • Monitors systems and identifies problem areas to Clinical Supervisor and Director of Nursing Operations both verbally and through written reports
  • Assists in investigating and reviewing initial employee and patient complaints as necessary and communicates to Clinical Supervisor and Administrator/Alt Administrator
  • Rotating on-call for clinical and staffing questions/concerns (including weekends and after hours)
  • Performs visits as necessary (including weekends and after hours)
  • Maintains professional knowledge and meets mandatory continuing education requirements of the agency/licensing board by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies
  • Assist with marketing events

Requirements:

  • Graduate of an accredited school of registered nursing. Diploma Program of Nursing or Associate Degree of Nursing or Baccalaureate Degree of Nursing or Baccalaureate Degree of Nursing or higher-level degree of nursing
  • Current Texas State Licensure as a Registered Nurse or Registered Nursing License from an approved Compact State with the Texas Board of Nursing (Will apply for Texas Nursing License within 30 days from date of hire) Communication planning time management organization and interpersonal skills
  • Demonstrated ability to supervise and direct professional administrative and direct care/clinical staff
  • Strong team player with the ability to facilitate team building develop effective partnerships and collaborate with a variety of stakeholders at all levels
  • Analytical skills
  • Maintains high level of confidentiality
  • Proficient in Microsoft Office
