Schedule: MondayFriday 8:30 AM5:00 PM EST (subject to business needs)
Location: Hybrid (Syracuse NY) 3 days onsite / 2 days remote following successful completion of onsite training
Were looking for a Patient Referral Coordinator who wants their work to mean something someone who brings care accountability and purpose to what they do every day.
The Patient Referral Coordinator plays a vital role in ensuring patients are ready for care clinically administratively and financially before services begin. By coordinating referrals accurately and efficiently this role helps remove barriers to treatment and supports a smooth patientfirst experience.

In this role you will:
- Ensure patient referrals are fully prepared for service by gathering required documentation confirming insurance coverage and securing authorizations in compliance with all regulatory requirements
- Coordinate new infusion and enteral referrals from intake through initial processing ensuring accuracy timeliness and clarity at every step
- Collaborate closely with referral sources hospital liaisons pharmacy nursing billing and branch teams to support seamless care delivery
- Communicate coverage details and financial obligations clearly to patients or their representatives ensuring understanding and agreement prior to service
- Maintain accurate patient records referral logs and authorization tracking to support timely billing and reduce delays in reimbursement
- Identify opportunities to improve intake and referral workflows while maintaining high standards of service and compliance

Qualifications
- Associates degree in Business Healthcare Services or a related field required
- Minimum of one (1) year of healthcare experience required
- Strong knowledge of payer requirements related to billing authorizations and coverage
- Proficiency with computer systems and documentation accuracy
- Strong organizational multitasking and problemresolution skills
Preferred
- Two (2) or more years of experience with a home care or infusion provider
- Prior home infusion experience on the provider or payer side

Pay Range/Rate
$22 - $26 USD
