Patient Navigator Physician Services Part time

Under the Direct Supervision of the VP of Physician Services the Patient Navigator willwork in collaboration and continuous partnership with patients and their family/caregiver(s)clinic/hospital/specialty providers and staff and community resources to connect patients to relevantcommunity resources with the goal of enhancing patient health well-being decreasing patientreadmissions and increasing patient satisfaction.

MINIMUM EDUCATION:

High School Graduate or GED Equivalent; relevant experience in lieu of High School Diploma/GED education will be considered.

MINIMUM WORK EXPERIENCE:

3 years previous customer service experience.

REQUIRED SKILLS KNOWLEDGE AND ABILITIES:

Previous experience in a customer support role. Strong phone andverbal communication skills along with active listening. Customer focusand adaptability to different personality types. Ability to multi-taskpriorities and manage time effectively. Ability to establish and maintainstrong working relationships with staff and management; excellentorganizational skills with emphasis on detail and ability to effectivelycommunicate both verbally and in writing. Detailed oriented teamplayer and an independent thinker.
