As a Patient Navigator youll serve as a trusted guide for patients and families helping them navigate the healthcare system access critical resources and overcome barriers to this highly collaborative role youll act as a key liaison between patients providers and community partners to ensure seamless patient-centered care and improved outcomes.

What Youll Do
Advocate for patients and families ensuring their needs goals and preferences are reflected in care plans.
Serve as a primary point of contact between patients providers and other healthcare stakeholders.
Coordinate appointments services referrals and follow-up care across multidisciplinary teams.
Help patients understand their care plans and support adherence to recommended treatments.
Connect patients with valuable healthcare and community resources including financial assistance transportation and support services.
Educate patients and caregivers on medical conditions treatment options and self-management strategies.
Empower patients to make informed decisions and actively participate in their healthcare journey.
Maintain accurate documentation of patient interactions care plans and resource referrals.

This role does not provide direct patient care but is instrumental in supporting patients and families through every step of their healthcare experience.

Qualifications:

• Bachelors Degree in related field of study required; relevant experience can be considered in lieu of degree.

• 1 years of patient facing medical office or other related experience strongly preferred.

• Bilingual Spanish speaking strongly preferred.

Additional Skills for Success:

• Strong knowledge of healthcare resources community services and patient advocacy.

• Excellent communication and interpersonal skills.

• Ability to collaborate effectively with healthcare professionals across multiple disciplines and experiences.

• Exceptional organizational and time management skills.

• Familiarity with electronic health records and case management software.

Schedule and Work Model

• Full-time Monday through Friday approximately 8am-4:30pm.

• East Region must be flexible to support Boston sites and practices in East Region

• Hybrid work model: On-site at Massachusetts General Medical Group (Staniford Street in Boston MA) 4 days per week; remote/work from home 1 day per week.

• Remote workdays require stable secure quiet compliant workspace with MGB issued equipment.
