Key Responsibilities

Patient Engagement

• Serve as a primary point of contact for patients building trust and rapport
• Proactively reach out to patients to assess needs and ensure they feel supported
• Educate patients on available programs services and resources

Referrals & Care Coordination

• Identify patients who would benefit from additional services and facilitate appropriate referrals
• Coordinate with internal teams and external providers to ensure smooth transitions of care
• Track referral outcomes and follow up to confirm patients are connected and engaged

Documentation & Reporting

• Maintain accurate and up-to-date patient records and interaction logs
• Track engagement activity and referral data to support reporting and quality improvement
• Ensure all documentation meets relevant compliance and privacy standards

Qualifications

• Experience in a patient-facing community health or care coordination role
• Strong interpersonal and communication skills with a compassionate patient-first approach
• Ability to manage multiple cases and priorities simultaneously
• Familiarity with healthcare systems referral processes or social determinants of health is a plus
