Job Summary

ThePatient Care Liaisonis a cross-functional healthcare support professional responsible for enhancing patient access supporting medical workflows assisting with continuity of care activities and maintaining accurate documentation within a federally aligned primary care clinic. This position serves as a vital connection between patients providers clinical staff and external healthcare organizations to ensure a seamless patient-centered healthcare experience.
The Patient Care Liaison combines administrative patient service and clinical support functions while helping patients navigate healthcare services and supporting providers in delivering high-quality coordinated care.
Duties to include:
  • Welcome and assist patients in a professional compassionate and confidential manner.
  • Schedule confirm and coordinate patient appointments.
  • Verify demographic insurance and registration information.
  • Coordinate referrals to specialty providers and community resources.
  • Track referral status and obtain consultation reports.
  • Prepare patient charts and ensure documentation is complete prior to appointments.
  • Support providers with administrative and clinical workflow tasks.
  • Ensure patient information is documented accurately and timely.
  • Maintain confidentiality and comply with HIPAA regulations.
  • Serve as a liaison between patients providers behavioral health staff nursing care management and external healthcare organizations.
  • Participate in team meetings and collaborate to improve patient outcomes.
  • Support clinic operations by assisting with special projects and other duties as assigned.
Minimum Requirements:
High school diploma or equivalent required Medical Assistant certification preferred. Minimum of 2 years experience in: Medical office operations Front desk registration Clinical support or Medical Assisting. Experience in using electronic medical record systems required. Strong organizational and multitasking abilities. Exceptional attention to detail. Strong communication and customer service skills. Ability to work independently and collaboratively. Understanding patient confidentiality standards. Attention to detail. Documentation accuracy. Patient-centered communication. Strong organizational skills. Professionalism and confidentiality. Workflow accountability. Time management. Problem-solving and critical-thinking skills. Ability to work independently and collaboratively. Ability to function effectively in a fast-paced highly regulated healthcare environment.
