Your work enables earlier clinical intervention and helps prevent avoidable hospitalizations directly advancing Circadias mission to bring high-quality proactive care into patients homes.
- Take and record patient vitals during home visits ensuring accurate readings are captured and transmitted through the Circadia platform
- Oversee installation of Circadia devices at designated patient locations for our Home Health Program
- Facilitate patients initial telehealth visits with our dedicated Home Health Nurse Practitioner
- Educate and empower patients to build comfort using the device and setting up telehealth visits
- Manage routine device maintenance IT setup configuration and assembly of medical equipment
- Coordinate with internal teams throughout the customer lifecycle to ensure successful technology deployment
- Audit and maintain the asset management list
- Support the Procurement Team with supply chain planning for future installs and company growth
- Come into the office once a week or as needed to pick up supplies
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification required with demonstrated experience taking and recording vital signs (blood pressure pulse respiration temperature pulse oximetry)
- Skilled with power tools and equipment for mounting and installation
- Valid drivers license and reliable transportation (required for daily travel between patient homes)
- Comfortable working independently with minimal supervision
- Strong communication and customer service skills especially with elderly or vulnerable populations
- Familiarity with HIPAA and patient privacy standards
- Willingness to pass a background check
- Mission-driven and passionate about helping patients get the proactive care they need
- Bilingual (Spanish) a plus given the Orange County service area
As a full-time Patient Care Coordinator you will be employed by Circadia Health Inc. The anticipated hourly range for this part-time position is $30-$35/hour. The hourly rate is determined by role and level and placement within the range will depend on a number of job-related factors including but not limited to your skills qualifications experience and location.
