Position Summary

As aCare Coordinatoryoullserve as the central connection between referral sources patients families physicians and our clinical team.Youllhelp coordinate the patient journey from referral through admission while ensuringtimelycommunicationaccuratedocumentation insurance verification and seamless scheduling.

Your work helps ensure patients receive the care they need when they need it making you an essential part of our mission to deliver exceptional home health services.

Responsibilities

  • Receive and process referrals from hospitals physicians facilities and community partners.

  • Verify patient demographics insurance coverage authorizations physician orders and required documentation.

  • Coordinate Start of Care visits within requiredtime frames.

  • Communicate with patients and familiesregardingservices scheduling and expectations.

  • Collaborate with nurses therapists scheduling billing physicians and referral sources to ensure seamless care coordination.

  • Maintainaccuratedocumentation within the electronic medical record (EMR).

  • Monitor referral progress and proactively resolve barriers to timely admission.

  • Assistwith scheduling adjustments and coordination across clinical disciplines.

  • Maintain compliance with HIPAA Medicare Medicaid and agency policies.

  • Provide exceptional customer service while creating a welcoming experience for every patient and referral source.

Qualifications

  • High school diploma or equivalent preferred.

  • One to two years of healthcare intake scheduling care coordination or administrative experience preferred.

  • Strong organizational and multitasking skills.

  • Excellent communication and customer service abilities.

  • Proficiencywith Microsoft Office (Outlook Teams Word Excel and PowerPoint).

  • Comfortable learning electronic medical record (EMR) systems.

  • Ability to prioritize competing responsibilities while maintaining attention to detail.

  • Passion for helping others and supporting exceptional patient care.
