Our dedicated team-clinicians nurses therapists and support staff-works collaboratively to provide comprehensive services that promote healing and recovery.

What This Role Looks Like

As a Case Manager in the PATH (Projects for Assistance in Transitioning from Homelessness) program you will play a vital role in helping individuals secure stable housing and access essential community supports. Your work will include:

  • Building relationships with clients and assisting them in navigating housing options

  • Connecting individuals to entitlements benefits and supportive services

  • Conducting community outreach to identify people who may qualify for PATH services

  • Collaborating with service providers treatment teams and natural supports

  • Participating in the development of Individual Recovery Plans (IRPs)

  • Providing interventions that promote empowerment independence and selfadvocacy

  • Using Motivational Interviewing (MI) to effectively engage and support clients

Your efforts will help remove barriers strengthen recovery and create pathways toward longterm stability.

What Youll Bring

To be considered for this role you should have:

  • A Bachelors Degree in a mental healthrelated field from an accredited institution

  • Two years of relevant experience

  • Strong knowledge of community resources and local agencies

  • Experience advocating for supportive services (preferred)

  • Excellent communication and engagement skills

  • Spanishspeaking ability (preferred)

  • A valid drivers license with an MVR that meets agency standards
