Description

Bay State Community Services is looking for an individual who is grounded in peer-assisted recovery and has valuable life experiences that provides expertise to the program.


What You Will Be Doing to Make a Difference...

  • Conduct home visits to provide parenting education and support focusing on child development early learning and family well-being.
  • Develop and implement individualized family support plans based on assessments and family needs.
  • Facilitate group meetings and workshops for parents on various topics including child development positive discipline and family health.
  • Use the PAT curriculum and materials effectively to engage families and promote developmental milestones.
  • Maintain accurate and confidential records of home visits family interactions and progress.
  • Collaborate with community resources organizations and professionals to connect families with additional support services.
  • Stay current with PAT guidelines and best practices through ongoing professional development and training.
  • Driving is needed

Location: Office is out of 400 Washington St Braintree but catchment is Brockton and Surrounding towns

Requirements

Qualifications/Credentials

  • High School Diploma/GED or above required
  • Lived experience in parenting is required
