What Youll Do

As an Outreach Worker in the FIT program you will provide high-quality community-based services to youth with Serious Emotional Disturbance (SED) and their families. You will work as part of a multidisciplinary team to deliver intensive treatment support and skill building to help stabilize youth in their home and community settings.

• Deliver intensive in-home and community-based services 35 times per week as clinically indicated
• Teach and coach youth in coping skills self-care and strategies for managing emotions
• Support families in addressing the emotional and mental health needs of youth
• Collaborate in the development and implementation of individualized treatment plans
• Provide behavioral coaching to enhance problem-solving communication and family dynamics
• Facilitate linkages to community resources and make referrals through warm hand-offs
• Co-facilitate team meetings and assist with aftercare planning

Who You Are

• Associates or Bachelors degree in Human Services with relevant experience or enrolled in a Masters degree program
• Experience providing therapeutic support to youth and families preferred
• Bilingual In Spanish (Required)
• Knowledge of mental illness diagnoses behavioral modification techniques and coping strategies
• Familiarity with child-serving systems and community resources
• Strong communication time management and organizational skills
• Ability to work with diverse populations and demonstrate cultural competence
• Valid drivers license and reliable transportation required
