The Outreach andAccess Specialist with the Innovative Healthcare Collaborative of Indiana LLC (IHCI) supports population health and attribution initiatives through proactive patient outreach and scheduling efforts. This role helps connect patients to their attributed providers improve continuity of care close care gaps and increase access to preventive services across the network.
Working collaboratively with care teams access centers and internal partners this position plays a key role in improving quality outcomes and enhancing the overall patient experience.
Key responsibilities:
Conduct high-volume outbound patient outreach to schedule Annual Wellness Visits (AWVs) preventive care services and primary care appointments.
Accurately document outreach activities scheduling outcomes and patient interactions within the electronic medical record.
Educate patients regarding preventive care services continuity of care and available access options.
Identify and help address barriers that may impact patient scheduling and engagement.
Meet established productivity scheduling and quality performance goals.
Collaborate with care teams access centers and internal stakeholders to ensure coordinated patient care.
Exceptional Skills and Qualifications
Applicants for this position should be able to collaborate with others in a team setting have excellent communication skills and a positive attitude toward problem-solving.
High School Diploma or GED required.
Minimum one (1) year of experience in healthcare patient access scheduling or customer service.
Minimum one (1) year of experience managing high-volume outbound calls or outreach activities.
Minimum one (1) year of experience utilizing Epic or a comparable electronic health record (EHR) system.
Strong organizational communication and customer service skills.
Ability to multitask and prioritize work in a fast-paced environment.
This position will allow flexibility to work remotely. Community caregivers performing work remotely are permitted to live in the following states: Indiana Illinois Ohio Michigan Kentucky Florida and Texas. Caregivers are not allowed to perform work remotely outside of the above states. Applicants from other states may apply; however if hired they will be required to relocate to one of the above states within 60 days of their employment date.
