Job Summary
Position Purpose
Under the direction of the Cardiovascular Service Line Nurse Director and the CAD Program Coordinator the Outpatient Cardiac Services Registered Nurse (RN) collaborates within a multidisciplinary team to deliver safe effective and patient-centered care.
This role supports guides and educates patients and their families throughout the surgical process across the continuum of care. The RN assesses patient needs develops and implements individualized care plans and evaluates patient responses to achieve desired outcomes. The RN also serves as an advocate assisting patients and families in navigating the complexities of the healthcare system.
What you will do
In this role you will work as part of a collaborative outpatient cardiac team ensuring high-quality care delivery and continuity of care. Key responsibilities include:
Assess plan implement and evaluate individualized patient care plans
Educate patients and families on procedures recovery expectations and ongoing care needs
Serve as a patient advocate and care navigator throughout the surgical continuum
Communicate effectively with interdisciplinary team members patients and families
Support quality improvement initiatives that enhance patient outcomes
Utilize electronic medical record systems and clinical documentation tools
Demonstrate teamwork through effective problem solving communication and conflict resolution
Education: Graduate of an accredited college or university affiliated nursing program. BSN preferred.
Licensure: Current Licensure as a Registered Nurse (RN) in Connecticut.
Certification: BLS (AHA/ARC).
Experience: Minimum of 3 years of nursing experience within a cardiovascular surgical setting or related specialty. Experience working with a cardiovascular surgical team preferred.
Skills: Basic computer proficiency required. Strong communication organizational and critical thinking skills.
Preferred Skills
Education: Bachelor of Science in Nursing (BSN) degree preferred.
Certification: ACLS
Experience: Experience in outpatient cardiac services care coordination or patient navigation strongly preferred.
Work Schedule
Part Time 24hr Day Shift Weekend and Holiday Rotation Required
