Concept Rehab is looking for a compassionate and motivated Occupational Therapy Assistant (OTA) to join our team on a PRN/as-needed basis in Cedarburg WI.

• Provide occupational therapy treatments under the supervision of a licensed Occupational Therapist
• Help patients improve strength mobility cognition functional skills and independence with daily activities
• Follow individualized plans of care and monitor patient progress
• Communicate with patients families nursing staff physicians and other members of the care team
• Participate in care plan and interdisciplinary team meetings
• Provide education and training to patients families and facility staff
• Complete accurate and timely clinical documentation
• Help patients achieve greater independence and quality of life

• Graduate of an accredited Occupational Therapy Assistant program
• Current Wisconsin OTA license/certification or ability to obtain
• Strong communication and interpersonal skills
• Dependable compassionate and patient-focused
• Previous skilled nursing or rehabilitation experience is a plus
