Job Summary
We are seeking a skilled and compassionate Occupational Therapist to join a Skilled Nursing Facility in Manteca California. This role focuses on helping patients regain independence in daily living activities following illness injury or surgery. You will work with a geriatric and post-acute population developing personalized treatment plans that improve functional ability and overall quality of life. Job Responsibilities:
Evaluate and treat patients with physical cognitive and functional limitations in a Skilled Nursing Facility setting
Develop and implement individualized occupational therapy treatment plans
Train patients in activities of daily living (ADLs) such as dressing bathing and feeding
Recommend adaptive equipment and environmental modifications to improve independence
Monitor patient progress and adjust treatment plans as needed
Maintain accurate and compliant documentation of patient care and outcomes

Requirements
Active state license as an Occupational Therapist in California
Current BLS/CPR certification
Minimum of 1 year of recent OT experience preferred ideally in a SNF or geriatric setting
Strong problem-solving and patient care skills
Ability to work collaboratively within a multidisciplinary team

Required Skills:
Job Requirements: Active state license as a Physical Therapist Current BLS/CPR certification preferred 01 year of experience welcome (new graduates encouraged to apply) Experience or clinical exposure in SNF rehabilitation or geriatric care is a plus Strong clinical reasoning and patient care skills Ability to work collaboratively within an interdisciplinary team
