About the Role
We are seeking an Occupational Therapist for a 13-week contract opportunity in Santa Rosa California. This role supports rehabilitation services in a skilled nursing facility setting and focuses on helping patients improve independence mobility daily living skills and overall functional ability.
Responsibilities
Evaluate patients and develop individualized occupational therapy treatment plans
Provide therapy to improve ADLs mobility strength coordination and independence
Support rehabilitation goals in a skilled nursing facility setting
Document patient progress treatment response and care plan updates accurately
Collaborate with nursing therapy teams patients and families to support recovery goals
Requirements
Active California Occupational Therapist license required
Minimum 1 year of Occupational Therapy experience required
Skilled Nursing Facility experience preferred
Must be available for variable schedule as assigned
Strong patient care documentation communication and rehabilitation skills required
