We are seeking a skilled and experienced Occupational Therapist (OT) to join our team as a Full-Time Home Health Therapist. The ideal candidate will have experience in home health and be able to provide quality care to patients in their homes.
Responsibilities:
- Assess patients needs and develop individualized treatment plans
- Provide therapy services to patients in their homes
- Monitor patients progress and adjust treatment plans as needed
- Collaborate with other healthcare professionals to ensure comprehensive care
- Document patient progress and maintain accurate records
- Provide education to patients and their families on therapy techniques and home exercises
Requirements:
- Valid Occupational Therapist (OT) license
- Experience in home health
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and critical thinking skills
- Ability to travel to patients homes
