We are currently seeking a compassionate and motivated Occupational Therapist (OT) to join our team in a skilled nursing facility in Linden Michigan.

As an Occupational Therapist you will evaluate residents develop individualized treatment plans and provide evidence-based interventions that help patients regain independence and improve their quality of life. Youll work closely with an interdisciplinary team committed to delivering outstanding patient care.
- Evaluate residents and establish individualized treatment plans
- Provide skilled occupational therapy services in accordance with physician orders
- Collaborate with physicians nursing staff therapists families and interdisciplinary team members
- Complete accurate and timely documentation
- Educate patients families and caregivers on treatment plans and home programs
- Maintain compliance with federal state and company policies
- Current Occupational Therapist license in the State of Michigan
- Graduate of an accredited Occupational Therapy program
- Excellent communication and interpersonal skills
- Skilled Nursing Facility or post-acute experience is preferred but new graduates are encouraged to apply
- Passion for helping older adults achieve their highest level of independence
