As a Home Health Occupational Therapist youll have the opportunity to work with patients where progress matters mosttheir homes.

Youll help individuals regain the skills needed to safely perform everyday activities improve function adapt to physical limitations and maintain their independence.

• Complete occupational therapy evaluations reassessments and discharge visits
• Develop individualized treatment plans based on patient goals
• Provide skilled occupational therapy services in patients homes
• Assess home safety and recommend adaptive equipment and modifications
• Educate patients and caregivers on techniques that promote independence and safety
• Collaborate with physicians and interdisciplinary team members
• Maintain timely and accurate documentation

• Active Virginia Occupational Therapist License Required
• Valid Drivers License and Reliable Transportation
• Strong Communication and Documentation Skills
• Home Health Experience Preferred but Not Required
• Commitment to Delivering Exceptional Patient-Centered Care
