Occupational Therapist

Position Summary:

Acclaim Home Health is seeking a compassionate and motivated Occupational Therapist (OT) to join our growing home health team in Worcester Massachusetts. This is an excellent opportunity for a skilled clinician who is passionate about helping patients regain independence and improve their quality of life in the comfort of their own homes.

As a Home Health Occupational Therapist you will work one-on-one with patients to evaluate functional needs develop personalized treatment plans and provide therapeutic interventions that support daily living and mobility.

Responsibilities

• Evaluate patients functional status and occupational therapy needs in the home setting
• Develop individualized treatment plans and goals
• Provide therapeutic exercises and adaptive training
• Educate patients and caregivers on safety mobility and activities of daily living
• Recommend adaptive equipment and home modifications
• Collaborate with physicians nurses physical therapists speech therapists and caregivers
• Complete accurate and timely clinical documentation
• Monitor patient progress and adjust care plans as needed

Qualifications

• Current Massachusetts Occupational Therapist (OT) license
• Graduate of an accredited Occupational Therapy program
• Home health experience preferred but not required
• Strong communication and interpersonal skills
• Valid drivers license and reliable transportation
• Ability to work independently and manage time effectively
