Contract: 13 Weeks
Schedule: Sunday Thursday or Tuesday Saturday
Guaranteed Hours: 30 hours/week
Experience: Minimum 1 year required

About the Role
We are seeking an Occupational Therapist for a skilled nursing facility contract opportunity in Los Altos CA. This role supports patients with improving daily living skills functional independence mobility and overall quality of life.

Responsibilities

• Evaluate patient functional abilities and care needs

• Develop and implement individualized treatment plans

• Provide occupational therapy services in a skilled nursing setting

• Support ADL training mobility strengthening and adaptive techniques

• Educate patients and care teams on therapy goals and progress

• Maintain accurate therapy documentation

• Collaborate with interdisciplinary care teams

Requirements

• Active California Occupational Therapist License required

• Minimum 1 year of Occupational Therapy experience required

• Skilled nursing facility experience preferred

• Must be able to work Sunday Thursday or Tuesday Saturday schedule

• Strong communication documentation and patient care skills required
