Occupational Therapist (OT) Home Health
We are seeking a dedicated and compassionate Occupational Therapist to join our team in the Houston area. The ideal candidate will provide high-quality care to patients experiencing physical cognitive developmental or emotional challenges that impact their ability to perform daily activities. This position offers flexibility with full-time part-time permanent and temporary options available.
Key Responsibilities:
- Evaluate patients functional abilities limitations and needs through comprehensive assessments.
- Develop individualized treatment plans designed to improve patients independence and quality of life.
- Implement therapeutic interventions focused on activities of daily living (ADLs) instrumental activities of daily living (IADLs) cognitive skills upper extremity function and adaptive techniques.
- Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes.
- Educate patients families and caregivers on home exercise programs adaptive equipment safety strategies and techniques to maximize independence.
- Maintain accurate and timely documentation of evaluations treatments progress notes and discharge summaries in compliance with regulatory requirements.
- Collaborate effectively with physicians physical therapists speech-language pathologists nurses and other healthcare professionals to coordinate patient care.
- Participate in continuing education and professional development activities to remain current with best practices in occupational therapy.
Qualifications:
- Bachelors Masters or Doctorate degree in Occupational Therapy from an accredited program.
- Current state licensure or eligibility for licensure as an Occupational Therapist in Texas (must be obtained prior to employment).
- Previous home health experience preferred but not required; new graduates are welcome to apply.
- Strong verbal and written communication skills with the ability to establish rapport with patients and families.
Skills:
- Knowledge of rehabilitation principles related to activities of daily living cognitive function and adaptive equipment.
- Experience using electronic health records (EHR) systems.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
- Compassionate patient-centered approach to care with a commitment to improving patient outcomes.
