• $105000 - $117000 Depending on Experience

Job Responsibilities

• Monday Friday
• Flexible Schedule
• No Weekends
• Coordinated Routes to Limit Travel
• Coordinates total patient care by conducting comprehensive health evaluations monitoring the clients condition promoting sound preventive practices coordinating services and educational and training activities
• Submits clinical notes progress notes and other clinical record forms outlining the services rendered as indicated

Job Requirements

• Occupational Therapist in the State of Florida Required Upon Hire
• 1 Year of Institutional or Home Health Experience Preferred
• Must Have OASIS Experience Preferred
