Position Overview

We are seeking compassionate and motivated Occupational Therapists to join a growing Home Health and Assisted Living team throughout the Denver metro area. This role offers the opportunity to work independently while making a meaningful impact on patients quality of life in home and assisted living settings.

As an Occupational Therapist you will evaluate and treat patients experiencing physical cognitive and/or psychological impairments that impact their ability to safely perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs). You will develop individualized treatment plans focused on maximizing independence safety and functional outcomes.

Key Responsibilities
  • Perform comprehensive occupational therapy evaluations and develop individualized treatment plans
  • Assess patient deficits related to ADLs IADLs mobility cognition safety awareness and functional independence
  • Implement therapeutic interventions designed to improve patient participation and quality of life
  • Recommend adaptive equipment environmental modifications and compensatory strategies to support safe function within the home or assisted living environment
  • Collaborate with physicians nurses caregivers and interdisciplinary healthcare teams to ensure coordinated patient care
  • Complete timely and accurate clinical documentation and billing in accordance with organizational standards
  • Maintain productivity expectations while delivering high-quality patient care
  • Participate in team meetings staff education in-services and quality improvement initiatives
  • Support patient safety initiatives and proper use/maintenance of patient care equipment
  • Participate in mentoring and student education opportunities as applicable

Qualifications
Required
  • Degree in Occupational Therapy from an ACOTE-accredited program
  • Current Occupational Therapist (OT) license in the State of Colorado
  • Strong organizational interpersonal and communication skills
  • Effective written and verbal communication abilities
