As an Occupational Therapist with Pathways PACE youll work as a valued member of an interdisciplinary team that truly collaborates to help participants remain healthy independent and living in the community.

Youll have the opportunity to:

• Build lasting relationships with participants and their families.
• Evaluate functional abilities and develop individualized treatment plans.
• Recommend home modifications and adaptive equipment that improve safety and independence.
• Collaborate daily with physicians nurses physical therapists social workers and other disciplines.
• Help participants maintain meaningful daily activities and improve their quality of life.
• Play an important role in launching a new PACE program and helping establish best practices.

Required:

• Graduate of an accredited Occupational Therapy program
• Active Colorado Occupational Therapist license
• Current CPR certification
• Valid Colorado drivers license and proof of insurance
• One year of Occupational Therapy experience

Preferred:

• Experience in skilled nursing inpatient rehabilitation hospital home health or geriatric care
• Experience working with interdisciplinary teams
• An interest in preventative community-based care
